change of Address
If you have recently changed your address,
emergency contact numbers or your phone number could you please let the centre
know, so we can keep our files up to date.
If you have also updated immunisation records
could you please give the centre a copy of the updated immunisation records.
This is to ensure we have all of your child’s/
children’s records up to date.
